If you are a new patient to speed up your wait time, please feel free to print your new patient paperwork and have it filled out before you arrive for your appointment. If you choose not to complete the paperwork ahead of time, please arrive 30 mins before your appointment time so that you can fill out paperwork, validate your insurance and take care of any payments.

• Insurance will be validated at every appointment and co-pay must be paid prior to being seen.

• At Rhema we encourage our patients to take an active role in their care, we ask patients to do this by arriving 15 minutes early for all appointments, bringing all medications to every appointment and by taking notes about anything that pertains to your health concerns.
